Upon completion of the attached survey your name will be entered into a drawing to win either one $100 OfficeMax gift card or one of four $50 OfficeMax gift cards. Drawings will be held weekly; you will be notified by mail if you are a winner. You may enter each time you make a purchase at OfficeMax. You must be 18 years of age or older to participate in the drawing.
