Help! Need VA.com Perfect Dark Rebate Form ASAP

I totally forgot about my VA.com bankruptcy claim. I need to send in all pertinant stuff, which would include a copy of the rebate form offered by VA.com. Does anyone have that PDF file still on their computer? I need it like yesterday. Please reply if you have a link to it. Thanks!
