1-800-526-1841. It's all automated, give name, addy, etc. Then they asked Q's about allegeries, etc. They will send you info and a survey to complete for the phone card.

Good find! But that flonaze survey was LONGGGG and boring. They also said they send out the phone card 6-8 weeks after they receive a completed written survey from you.
