Does anyone know where I can download the rebate form? MS's page is totally unhelpful (surprise). It looks like they only let you view the rebate AFTER you buy it from them.

I was wondering the same thing. I just bought it from Staples and I really hope I can get the rebate, otherwise I'll be sending it back.

Well I checked out the link to Microsoft's webpage and its says the rebate is suppose to be in the product box. I'm guessing you just open the box and it should be there. This might be a problem since the rebate might have just been put out recently and if Staples has a lot of old boxes they might not be in there. Anyone know if this is true or not?

yeah...i don't see it.
but if you actually go into a B&M store (namely compusa or staples), they usually have a rebate center, where they have all the rebate forms a lot of stuff...

I called the number, and they said that if for some reason the rebate is not in the box, just call that number back (thanks Phil!) and they will send you one.

I got mine today and it did come with the rebate outside the plastic wrapper. The only thing inside was the CD which had Motocross Madness, drivers (on same CD), registration card and brief instruction card of Motocross Madness and using your SideWinder Freestyle Pro. First thing I suggest you do is use a fine felt tip when filling out the rebate. This stupid thing is like wax paper material so it takes about 5 minutes just to fill out your name, address and telephone #. It's like they purposely did it so you make it illegible to not get the rebate. Finally found something to replace my old Gravis Gamepad 4-button even though I haven't played a game with a joypad in ages let alone my Logitech Wingman Extreme. It's nice in that it comes with a USB converter if you already have a joystick plugged into the gameport.

So here is the scoop from Microsoft when a rebate card is not in the Sidewider box:
Send the following items:
Copy of Sales Receipt
Box top with barcode
Name
Address
Telephone number
Memo indicating that you did not receive a rebate in the box.
Send to:
Sidewinder Freetyle Pro
Promotions #49700664
P.O. Box 1128
Ridgely, MD 21681
They will send you a $30 check.

THanks for the heads up ona great deal. i just got mine today, with no rebate form in or on the box. I will follow the procedure listed above.
I still have one question: There was no receipt in the box, just a packing slip. Does Staples mail the receipt separately, or do I just print my receipt from the web site?

I ordered it on Friday, 8/4. Within a few hours I received an order confirmation stating it would be shipped within two days. According to the UPS label, it was shipped the same day (UPS ground).
