OM CD Stopmer rebate help!

I just bought CD Stomper from OfficeMAX, but in the rebate from, it says:
Company/Organization
(Check only paid to individuals)

What should I put there? Should I put my name there?

This is my first time see this in the rebate form.

Leave it blank. You should have already written your name on the form above that. Make copies of everything and write down the date you mail it. I usually keep a copy of the ad as well but I have never had to use it...yet. If you don't receive your rebate within the specified time, take your copies and go see the manager. Call if it is a dotcom order.
