need help with appication for an absentee ballot!!!

so I have an absentee ballot request form in front of me!

I have no idea where I have to send it too! It says I have to send it to my county elections office!

I'm in the San Joaquin County in CA. I've tried googling the address for the elections office for the San Joaquin county...but no luck!

do tell how I can send in my request form! This is my first time voting with absentee. I don't know how to do jack squat!!! do help ya'll
