You're probably thinking about it too hard. There's really nothing to "get", it's just a silly picture with a silly phrase.

I guess I think it's funny that the cow with "one tit" looks more like a steer, and it's funny to think of the guy trying to attach a milking machine to a steer.

(For you city-types: Cow = female bovine, steer = castrated male bovine.)
