Free Chiptole burrito for tax day

It's a bit complicated but thought I'd pass it along anyway.

Purchase a burrito on 4/14 or 4/15. Keep the receipt and staple it to Chiptole's "tax form" and bring it back to Chiptole on 4/16 to get a free burrito.

Where do you get the form? I am assuming at the store when you buy your burrito (or do you have to request it?)
