online form help

I am trying to put a form on line so i can have the form after it is filled out e-mailed to me. can someone recomend a free program for doing this. i tried to use the form program in Frontpage but it would not work properly for me.

the form has about 50 questions some check boxex, drop down selections fill in the blank etc.

what kind of server are you going to put this on? It needs to have support for cgi/perl, asp or php.
