I lost my immunization card...any ideas?

So i need to send my school health information, and especially my immunization records...but I lost my card.

I'm going to call my local doctor's office, but I'm not sure they'll have anything since I only went there a few times. I don't remember what doctors I went to previously

Check with your previous schools-they MUST

Every school you went to since Kindergarten HAS to keep a copy of your school health records..check with the last school you went to-they should have a complete copy of the records, but it may take time if they have to search a data base for them.

Show up at the school looking all haggard, with a fake dog bite and foam coming out of your mouth and tell them .."honest all my shots are up to date!"

Also, this may seem obvious so humor me but have you asked your parents if they have a copy or who your previous doctors were? If you had your medical files transferred to any new doctor he/she should also be able to make a new card for you based on that.

Called my doctor, and they don't have it. They say they don't get records from previous doctor unless they need it.

I did call my university and they do have it, but I'm not sure how long it takes them to fax it to me

But at least I have it.

Are you anywhere near your university? If time is of the essence it might behoove you to drive down there and pick it up yourself. University paper pushers aren't exactly known for being the most efficient, dedicated people in the world.

i wouldn't know where to look if i needed an immunization card. i swear i've never seen one. perhaps our family doctor just took care of all that when we were going to school.

I never had mine, But I am very diligent about keeping my kids up to date for state benefits.

Call your last dr, they should have a copy on file. It might cost you a few bucks, but at least you'll have it.

Edit: I didn't read the post where you said you already called the dr. Sorry. No Ideas for you other than that one.

heh My mom has mine, at least the last time I know of. Glad you are getting it, hope it is quick.
